Moving to an IP telephony system allows to take advantage of the distributed model, no dedicated link are required between various components of the installation, only IP. For sure the IP phone will remain next to the user, but all the infrastructure components (gateway, proxy, registrar, gatekeeper, SBC, …) could be located anywhere on the IP network. Why not think about an externalization of these components in a specific hosting facility?
If the complete solution is rented from a provider we generally call this one Centrex, the deal is really simple: a cost per month and per phone + communications. But we can also talk about hosting of chosen equipment at various levels.
If such solution is in mind, the chosen telephony system should support hosting and quality control of the service through log and statistics. After, a simple infrastructure could be put in place in order to achieve IP telephony hosting, the main point is the network which should be resilient and provide expected performances.
Once technical issues are solved we can look at the other main point which is the cost. The main difference is moving from an invest model to a recurrent model which is really different for small companies that may like a cost linked to the number of phone versus a solution that can handle a certain amount of phones but has more capacity than really used.
